You MUST complete this form to receive important classroom information. Submit one form for each student.
Student Directory: The PTA gives a school-wide Student Directory to each student, which provides contact information for every student at FAAM. You can choose to opt out of listing your contact information in the Student Directory by checking the box at the bottom of the page; therefore, only your child’s name will be listed but without any contact information you provide here. When you opt out, any information you provide will ONLY be included in the Classroom Directory.
Classroom Directory: This form is used to create a Classroom Directory for your child’s homeroom Room Parent(s) to contact you (primarily via e-mail) with important information, dates, field trips, parties, etc. If you do not provide at least an e-mail address, you will not receive important classroom information.
